Can doxycycline help reduce the risk of sexually transmitted infections?

Doxycycline is an antibiotic that has been studied for preventing bacterial sexually transmitted infections (STIs) when taken shortly after sex. This approach is called doxycycline post-exposure prophylaxis (Doxy-PEP). Research shows it can lower the chances of getting chlamydia, syphilis, and gonorrhea, especially in men who have sex with men (MSM) and transgender women (TGW). However, it is not a replacement for condoms or regular STI testing, and concerns about antibiotic resistance mean it should be used under medical guidance.

What the research says

A 2025 meta-analysis of 14 studies found that doxycycline prophylaxis (either taken before or after sex) reduced the risk of any bacterial STI by 60% 5. Specifically, it lowered chlamydia risk by 82%, gonorrhea by 39%, and syphilis by 80% 5. Another systematic review and meta-analysis of four randomized trials reported that Doxy-PEP reduced the risk of any bacterial STI by 46%, chlamydia by 65%, and syphilis by 77% 11. A landmark 2023 trial showed that among MSM and TGW taking HIV PrEP, Doxy-PEP cut STI rates from 31.9% to 10.7% per quarterly visit 9. A 2024 French trial (DOXYVAC) also found Doxy-PEP effective for preventing chlamydia and syphilis in MSM 10.
